Community First Services & Supports (CFSS) formerly PCA
Personal Care Assistance (PCA) services, now Community First Services & Supports (CFSS), is the non-medical part of our home care service. We serve recipients who opt for the agency model.
Assisted Living

Home Health
Our home health personnel services work under the direct supervision of a registered nurse. They provide assistance to patients who need basic/complex medical care and basic/complex home management services in their place of residence.
Waiver Services/Home & Community-Based Services (HCBS)
Waivered Services recipients “waive” certain federal requirements to allow states to offer more flexible services to specific populations, usually outside of institutional settings like hospitals or nursing homes.
Home Care Nursing (Private Duty Nursing)
In-home care allows your loved one to get the help he or she needs without giving up independence. If your loved one is healthy, you might be able to meet his or her needs on your own or hire someone for traditional in-home care.
Care Management Services
Care Management Services are provided either by a Registered Nurse (RN) or a Licensed Social Worker (LSW) who oversees the case, provides additional training and guidance to staff
Residential Services
We also provide services for these individuals that are delivered through a range of Medicaid waiver programs, including Developmental Disabilities (DD), Community Access for Disability Inclusion (CADI), Brain Injury (BI), and Community Alternative Care (CAC).
Companion / Caregiver
Companion/Caregiver service is companionship and assistance in activities such as meal and medicine preparation, transportation and completion of errands in the recipient’s home.
